If assistance is necessary for completing the forms, please contact the Greater Dayton Volunteer Lawyers Project or Legal Aid Line at the linked websites, or at (888) 534-1432.  The Clerk of Courts is unable to advise what forms should be filed and unable to assist with completing any of the forms.

 Please note: No handwritten forms will be accepted. To complete fillable forms, you will need to right-click on the document and download to your computer, or you can fill out the form and click save as to your computer. Any forms not fillable need to be printed out and typed.  Forms can be submitted via fax (937) 562-4010, via email to [email protected], in person, or mailed to Greene County Juvenile Court, 2100 Greene Way Blvd., Xenia, OH 45385.  Please Note: You must sign your documents and have them notarized before submitting to the Court. 

It is recommended to use Google Chrome or Internet Explorer when filling out our forms.  Otherwise, you will need to change your PDF reader to Adobe Acrobat in order to fill out the forms.
