FOR PRO SE LITIGANTS (Filing without an Attorney):

If assistance is necessary for completing the forms, please contact the Greater Dayton Volunteer Lawyers Project or Legal Aid Line at the linked websites, or at (888) 534-1432. The Clerk of Courts is unable to advise what forms should be filed and unable to assist with completing any of the forms.

The following Pro Se Resources are Available at the Juvenile Court:

  1. Pro Se Self-Help Kiosks- two computer kiosks with printer access are available on the upper level of the juvenile court just outside of the Clerk's Office to provide litigants filing on their own with computer and printer access.
  2. Pro Se Clinic in Partnership with Greater Dayton Volunteer Lawyers Project (Monthly)- clinic hours are now available for litigants to take advantage of pro-bono (free) legal services for civil matters, to include assistance completing forms. The clinic will open on February 8, 2024 from 3:00-6:00 p.m. and will take place the second Thursday of every month for the remainder of the year. If you would like to take advantage of these services and be placed on the clinic schedule, you must first contact the GDVLP at (937) 461-3857 to go through the intake and income eligibility screening process. An online form is also available to register with GDVLP at this link []. Once you fill out the registration form GDVLP will contact you to complete the full intake process and get you scheduled for an upcoming clinic.  

Please note: No handwritten forms will be accepted. To complete fillable forms, you will need to right-click on the document and download to your computer, or you can fill out the form and click "save as" to your computer. Forms can be submitted via fax (937) 562-4010, via email to [email protected], in person, or mailed to Greene County Juvenile Court, 2100 Greene Way Blvd., Xenia, OH 45385. 

It is recommended to use Google Chrome or Internet Explorer when filling out our forms. Otherwise, you will need to change your PDF reader to Adobe Acrobat in order to fill out the forms.

Please Note: You must sign your documents and have them notarized before submitting to the Court if you are not submitting them in-person
